What is GUIDE?

The GUIDE Program was created to improve the quality of life for individuals living with dementia while helping them safely remain at home and reducing the burden on family caregivers.

Eligible individuals may receive:

  • Ongoing support and check-ins
  • Resource referrals
  • Care coordination

Respite Care

Qualified families may receive up to $2,625 annually toward in-home respite care, adult day services, and other approved support.

Medical Oversight

  • Telehealth support
  • 24/7 access to specialty providers
  • Coordination with healthcare professionals

Who May Qualify?

The GUIDE Program is available for eligible Traditional Medicare beneficiaries living with dementia who meet Medicare’s program requirements.
