Starting Your Recovery Journey After a Hospital Discharge to Adjust to Your New Normal

Hospital stays, especially unexpected ones, can be physically and emotionally challenging for patients and their families. While a hospital discharge marks the end of in-hospital care, for many patients, it signals the beginning of a new chapter in their recovery journey.

Adjusting to life after a hospital stay can be daunting, especially when facing a “new normal.” Whether it involves physical therapy, managing new medications, following a strict diet, or attending follow-up appointments with various healthcare providers, the period of time immediately following a hospital discharge may cause feelings of uncertainty or apprehension for all those involved.

How Post-Hospital Syndrome Affects Patients

This experience, known as post-hospital syndrome, can be a particularly vulnerable time for patients. During this period, patients are at an increased risk for hospital readmission due to a variety of conditions, often unrelated to the initial reason for hospitalization. That’s why having the right support system in place is crucial for a smooth and safe recovery.

Five Tips for Navigating a Hospital Discharge

Here are five tips on navigating a hospital discharge to help ease the transition from hospital to home and help reduce the risk of readmission.

  1. EDUCATION – Understanding your condition, treatment plan, and what to expect after discharge is crucial. Always ask your healthcare team to explain any medical terms, procedures, or medications you are unsure about. The more informed you are, the more confident you’ll feel in managing your recovery at home.
  2. COMMUNICATE EFFECTIVELY – Keep an open line of communication with your healthcare providers, family, and support network. Don’t hesitate to reach out if you have questions or concerns about your recovery. Clear communication can help prevent misunderstandings and ensure you receive the care and support you need.
  3. PRIORITIZE REST AND NUTRITION – Your body will likely need time to recuperate and regain energy after a hospital stay. Create a calm and restful environment at home and prioritize getting enough sleep to aid your recovery. Proper nutrition is equally important and will provide the strength and energy needed for a successful recovery.
  4. KNOW YOUR LIMITATIONS & ASK FOR HELP – Recognize that you may have some limitations after your hospital stay. You may need additional support for tasks you could easily do before your hospital stay, such as bathing, dressing, housekeeping, or preparing meals. Understanding your current abilities and gradually working towards your recovery goals can help prevent setbacks and reduce feelings of frustration. Lean on your support network so you can avoid overexertion and focus on getting better.
  5. BE PATIENT – Recovery is a journey that takes time. Depending on your condition, it can last weeks, months, or even longer after discharge. Be patient with yourself and give your body the time it needs to heal.

While hospitals have traditionally been the focal point for reducing readmissions, many healthcare services along the care continuum can do their part as well, including in-home care.
