Message from our Care Manager - Christina Johnson
As a Care Manager, I am dedicated to supporting client’s and their families with compassion, respect, and reliability. I ensure that each individual’s unique needs, preferences, and goals are honored. My focus is on creating a safe, comfortable, and dignified environment where seniors can thrive, while also providing families with peace of mind. With experience in coordinating care, advocating for clients, and guiding families through the challenges of aging, I strive to be both a trusted resource and a caring partner. Whether it’s managing daily needs, navigating healthcare, or simply offering a steady presence, I am committed to making a positive difference in the lives of those I serve.

We are pleased to introduce Buena Vista Home Health & Hospice in Thousand Oaks. Their experienced and compassionate team provides a full continuum of care, including Home Health, Palliative Care, and Hospice Services. With a focus on comfort, dignity, and personalized support, they help patients and families navigate every stage of care with warmth and respect.

RISE AND REST PACKAGE
We're excited to unveil our Rise & Rest package, designed to give seniors the perfect start and finish to their day.
Get essential support during the two most vulnerable parts of the day with a savings of $50-100
One 1-Hour A.M. shift and one 1-Hour P.M. shift = $120/day
One 2-Hour A.M. shift and one 2-Hour P.M. shift = $180/day
OUR NEW PACKAGE OFFERS
Rise Visit
Helping with tasks like wake-up assistance, dressing and hygiene, light meal prep, medication reminder, and light housekeeping.
Rest Visit
To wind down with assistance such as bedtime preparation, pajama change, bathroom support, and medication reminders.
HONORING OUR VETERANS WITH CARE
As a VA-credentialed provider, we proudly assist Veterans and surviving spouses with respectful, in-home care. Our caregivers offer personal support such as bathing and grooming, companionship, meal preparation, medication reminders, light housekeeping, transportation to appointments, and respite for family caregivers. If you or a loved one may qualify for VA benefits - including Aid & Attendance or the VA Homemaker/Home Health Aide program - we can guide you through the process and help start services quickly.
