This program provides flexibility and choice, giving participants the power to design and direct the services that best support independence, dignity, and community living.

What is the Self-Determination Program?

The Self-Determination Program (SDP) is available to all eligible Regional Center participants and their families. It allows individuals to have:
  • More flexibility in designing supports.
  • More choice in selecting service providers (vendored and non-vendored).
  • More control over how their budget is spent.
We work closely with each family to identify immediate needs and design a program that meets their Person-Centered Plan (PCP) goals.

Services We Provide Under SDP

310 – Respite Services

Relief care for families, giving parents or guardians a break while their child or adult loved one receives safe, compassionate support. Example: A parent of a child with autism uses respite hours on weekends so they can rest, while their child enjoys structured playtime with a trained caregiver.

312 – Live-In Caregiver

For individuals who need consistent support at home, a live-in caregiver provides 24/7 help with daily activities, safety, and companionship. Example: A young adult with significant physical disabilities has a live-in caregiver who assists with morning routines, meals, and nighttime safety.

313 – Homemaker

Help with household tasks so participants can live in a safe and organized environment. Services include laundry, cleaning, meal prep, and errands. Example: A teen with developmental disabilities practices cooking simple meals while a homemaker assists with shopping and maintaining a clean home.

320 – Community Living Supports (Independent Living Skills)

Skill-building to enhance independence and self-sufficiency. Training covers cooking, budgeting, using public transportation, personal hygiene, and other daily life activities. Example: A participant learns to create a weekly budget, shop for groceries, and prepare simple dinners with coaching from a caregiver.

331 – Community Integration

Activities designed to build confidence and connection in community settings. Supports may include attending local events, volunteering, joining art or sports programs, or practicing public transit. Example: A young adult attends a community college class with support, learning both academic and social skills while navigating campus life.

338 – Non-Medical Transportation

Safe, reliable transportation so participants can access school, work, day programs, or community activities. Can be used alone or combined with other services. Example: A participant takes agency-provided rides to a vocational training program and volunteer shifts each week.

371 – Caregiver Training & Counseling Services

Education and counseling for family members or paid caregivers. Training covers behavior management, safety, communication, and coping strategies for caregiver stress. Example: Parents of a child with challenging behaviors receive coaching on positive reinforcement techniques and attend counseling sessions to manage stress.

Steps to Care

  1. INITIAL CALL – Once we’ve received your information, we will have a detailed call to gather more information on the care situation, answer any pressing concerns, and set up a time to meet in person.

  2. IN-HOME CARE ASSESSMENT – Here we will answer all your questions, and ask some of our own to craft a Cared-4℠ Customized Care Plan to your unique needs.

  3. START OF CARE – We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.

  4. CLIENT VISITS – Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
