Our Safe Journey Home Program provides Manatee County residents with a smooth, stress-free transition from hospital or rehab to home.

We coordinate safe general transport or medical transport, and provide 5 hours of caregiving services, starting approximately 15 minutes prior to the planned discharge time.

What’s Included:

• Caregiver arrival about 15 minutes before discharge
• 5 hours of caregiving support starting when the caregiver arrives
• Help with transfers, personal care, prescription pickup, meal prep, and more
• Additional care can be arranged at our standard hourly rate

How It Works:

1. Submit the form below or call (941) 999-1960.
2. We coordinate directly with Drive Care or other third-party transportation partners for transportation.
3. Your caregiver meets you at the facility and follows you home.
4. You receive 5 hours of support to get settled safely and comfortably.
