Our Safe Journey Home Program provides Manatee County residents with a smooth, stress-free transition from hospital or rehab to home.

We coordinate safe medical transportation through our trusted partner, Drive Care, and provide 5 hours of caregiving services, starting approximately 15 minutes prior to the planned discharge time. Your caregiver will meet you at the facility, assist with gathering belongings and transfers, follow the transport vehicle home, and stay to help you settle in comfortably.

What’s Included:

• Caregiver arrival about 15 minutes before discharge
• 5 hours of caregiving support starting when the caregiver arrives
• Help with transfers, personal care, prescription pickup, meal prep, and more
• Additional care can be arranged at our standard hourly rate
