Key Steps to Access VA CCN In-Home Care Services

Enrollment in the VA Health Care System

  • Application: Veterans need to apply for enrollment in the VA health care system using the 10-10EZ form. This form is the starting point for applying for VA health benefits, including geriatrics and extended care.
  • Submission: Submit the form online, in person, by mail, or by telephone to a VA health care facility.
  • Processing: Processing takes approximately one week. After that, veterans will receive an approval letter and a call to help establish their care.

  • Authorization: The HHA Program Support Assistant will place the veteran with a credentialed HHA provider. Then, the HHA Program Support Assistant will authorize the provision of services.
  • Scheduling: The selected provider will coordinate with the veteran to schedule the in-home care services.
