In-Home Supportive Services, or IHSS, and private home care can both help a person remain at home, but they are arranged and paid for differently. IHSS is a California public program for eligible people with a Medi-Cal determination. The County assesses which tasks and monthly hours it will authorize. Private home care is arranged directly with a licensed provider and paid privately unless another verified funding source applies.

For a San Diego family, the decision is not always one program or the other. The useful question is which hours IHSS may cover, who will manage the caregiver, and what help remains outside the authorized plan.

What IHSS approval covers

The California Department of Social Services IHSS program page says applicants must live in California, have a Medi-Cal eligibility determination, live in their own home or another qualifying residence, and submit a completed health care certification. A county social worker then assesses the tasks the person cannot perform safely and determines which services and monthly hours to authorize.

San Diego County lists examples such as bathing, grooming, dressing, housecleaning, shopping, laundry, and meal preparation. Its local IHSS page also explains that approval is individualized and that a Notice of Action states the approved services, hours, and any share of cost.

IHSS is therefore not a standard block of hours that every recipient receives. The schedule follows the county decision. If needs change, the recipient should contact the assigned worker rather than assume additional time is covered.

Medi-Cal financial rules also change. The California Department of Health Care Services reports that asset rules changed on January 1, 2026, and are scheduled to change again in 2027. Families should use the current state and county guidance instead of relying on an older online threshold.

Who manages the IHSS caregiver

Under the usual individual-provider model, the recipient is considered the caregiver’s employer. San Diego County says the recipient is responsible for hiring, supervising, and, when necessary, dismissing the provider. The county’s IHSS provider page also states that providers complete enrollment requirements and a California Department of Justice background clearance.

A provider may be a relative, friend, or another enrolled individual. The Public Authority Registry can help a recipient look for an available provider, but the recipient still directs the day-to-day work. That responsibility matters. A family should decide who will explain routines, approve time sheets, address reliability concerns, and handle a provider’s absence.

What private home care changes

With private home care, the family contracts with an agency for a proposed schedule and service plan. California’s Home Care Services Branch licenses Home Care Organizations and maintains the Home Care Aide Registry. Families should still ask each agency how it screens caregivers, supervises services, handles schedule changes, and responds when the usual caregiver is unavailable. An agency structure does not make every schedule or replacement guaranteed.

Home Helpers Home Care of Clairemont provides local nonmedical services as including personal care and companionship, meal support, and social engagement. Specific tasks, timing, availability, and pricing can be confirmed during an assessment.

Private home care does not require an IHSS authorization. It also creates a separate private bill. Families should not assume an IHSS approval can be transferred to an agency invoice or that the county will reimburse private agency care.

How families may use both

Consider a hypothetical older adult in Clairemont whose IHSS hours cover several weekday morning routines. The family may still need help on two evenings or while the regular provider is unavailable. If the budget and agency availability allow, a separate private-pay schedule could cover those times.

Keep the two arrangements clear. Match each caregiver to specific days, hours, and tasks. Record which hours are authorized through IHSS and which are paid privately so the same time is not reported under both arrangements. Ask the county worker about any uncertainty involving the IHSS case.

This approach is useful only when the family can manage both schedules. Adding a second provider without a written weekly plan can create gaps, duplicate tasks, or confusion about who is expected to arrive.

A practical San Diego decision path

Use the benefit decision and the care schedule as two separate pieces of work:

  • Apply for IHSS or confirm the current Notice of Action with the County.
  • Mark the approved tasks and hours on a one-week calendar.
  • Identify mornings, evenings, weekends, or supervision needs that remain uncovered.
  • Decide whether the recipient and family can recruit and direct an individual provider.
  • Request a written private-care estimate for only the uncovered periods.
  • Compare the combined plan with the family’s budget and management capacity.

Do not delay necessary safety or medical help while waiting for a benefit decision. IHSS and private home care are nonmedical supports; urgent symptoms and clinical questions belong with the appropriate healthcare or emergency service.

Where to begin

For IHSS eligibility, application status, authorized tasks, or share-of-cost questions, begin with San Diego County IHSS. The County, not a private agency, determines eligibility and hours.

If you already know where the weekly gaps are, Home Helpers Home Care of Clairemont can discuss a separate private-pay schedule. You can schedule a care assessment to review the tasks and times under consideration. The conversation should clarify what private care could cover without replacing county benefits counseling.
