Steps to Free VA In-Home Care for Veterans through VA CCN
Are you a veteran seeking in-home care services? The VA Community Care Network (VA CCN) provides a great option for veterans care in the home for veterans of all ages. This applies whether you have a service-connected disability or not. In this blog post, I’ll explain how to start with VA CCN so you can get the care you deserve.
Submission: Submit the form online, in person, by mail, or by telephone to a VA health care facility.
Processing: Processing takes approximately one week. After that, veterans will receive an approval letter and a call to help establish their care.
Establishing Care with the VA
First Appointment: The VA assigns veterans a primary care physician (PCP). They will meet the clinical team, also known as the Patient Aligned Care Team (PACT). The Patient Aligned Care Team includes a doctor, social worker, and nurse.
Advocacy: Veterans should clearly express their need for care to ensure appropriate support levels.
Eligibility Assessment by VCCP
Evaluation: The Veterans Community Care Program (VCCP) will determine if the veteran’s disability is service-connected. The program will also determine if the veteran qualifies for community-based services.
Categories: Veterans are eligible for community care regardless of whether their disability is service-connected. Enrolled veterans are eligible for various VA programs and services.
Order for HHA Services
Clinical Coordinator: A clinical coordinator will assess the veteran’s needs and determine the appropriate number of care hours. This includes evaluating the need for a home health aide/personal care services.
Preferred Agency: Veterans can specify their preferred in-home care agency during this assessment.
Coordination and Provision of Services
Authorization: The HHA Program Support Assistant will place the veteran with a credentialed HHA provider. Then, the HHA Program Support Assistant will authorize the provision of services.
Scheduling: The selected provider will coordinate with the veteran to schedule the in-home care services.

"We have reached out to jonathan and his team 2 times when my father-in-law fell and needed extra care at home. Since we live out of state, its was a huge weight off of our shoulders to know that my miL had the support she needed.
